I, "the customer/client" understand:
My services will be provided in a safe and environmentally friendly setting. I release the practitioner of any liability that of the premises in which services/treatment are rendered.
I have disclosed or listed all pertinent information, to the best of my knowledge, regarding injuries, medication, and medical conditions that may affect my session/treatment.
I must arrive on time to receive my full service. If I arrive 15 minutes or more after the start of my appointment, my appointment may be canceled.
Late arrival, up to 15 min., will reduce remaining time for my service. No partial refund.
Second occurrence of late arrival or cancellation will require approval of any service.
Cancellation window is 24 hrs. cancellation prior to the start of service. After 24 hr. window, I am allowed one (1) opportunity to reschedule for a later date.
Appointment for clients only. Please do not bring children or other guests.
All guests/clients, ages 10-18 years, maybe serviced with parental consent and presence is required.
All services require deposit with remaining balance in person or payment online payment.
Payments and gratuity are accepted by third party processing such as PayPal, Zelle, AfterPay, Affirm, etc. Cash accepted - US dollar currency.
Discounts and rates may change without notice. Coupons, promotional offers, and eGifts must be provided at booking. Valid dates and value must be available. If expired, the value is redeemable towards service up to 30 days after expiration date.
If my service is terminated for any reason, no refund will be issued.
All guests are welcome regardless of gender or gender identity.
